A simple, field-by-field guide to the Department of Immigration & Emigration “Application for an Entry Visa” (Form B), with examples for every box.
Most short-stay tourists apply for Sri Lanka’s Electronic Travel Authorization (ETA) online instead of this paper form. Form B is usually used when applying through a Sri Lankan embassy or mission. Visa rules and fees change, so always check the Department of Immigration & Emigration and the official ETA website before you travel.
The form is 5 pages long. Pages 1–4 are for you to complete, and page 5 is for office use only. Have these ready before you begin:
